You are the clinicians eyes and ears
Things to always include in your note: (Primary Care)
Onset, Full dates
Track hospitals (primary care)
ER, hospital or urgent care visits
New medical history from other specialist
New medication from other specialist
And remember if the clinician that you are serving keeps asking you the same questons. Try to get that information for each patient if it applies. There is an art to serving and it lies in being proactive.
YOUR CLINICIAN IS YOUR CUSTOMER.
Someone once asked me: “In the medical field, who is your customer?” I replied: The patient, right?” WRONG! Your customer is the medical doctor, clinitian: nurse practitioner, physicians assistant that you serve everyday and the people who hired you.
PRO TIP: What goes on in the room during info gathering time DOES NOT STAY in the room. Try to minimize the convo to what you can document. I know it is not always possible but, try. Let the patient know they can speak to the doctor more about their concern when he or she comes in.
